New Patients

If you have received a referral for Physical Therapy or Occupational Therapy the fastest way to be seen is to head to the nearest office.  You will need to bring your ID and insurance cards with your referral and we will get you enrolled and authorized.  You can call us but sometimes we do not have a copy of your referral yet.   If you have a copy bring it down for faster service.  Otherwise, we will get to the faxed referrals as quick as we can and give you a call.   All of our forms are online and when you enroll with us you will be sent a link to fill out all your pertient information.  It is best to fill everything out as soon as you receive the link
